► Instructions: 
1. Type your facility name from the dropdown list below.
2. Tell us who you are, enter your name in the Contact Name field.
3. Put your phone number so we can call you with any questions.
4. On the equipment there should be a sticker that has an id number on it. It will have CBS Technologies on the sticker as well. Put that number in the Equipment ID field.
5. What type of equipment do you want us to repair, tell us in the Equipment Type field.
6. Where is the equipment at (dept.), That goes in the Location field.
7. Finally what is wrong with the equipment, tell us the problem.
8. That's it, click submit to send us this information and we will assign a work order number to the job.

We will call you to set up a time to arrive on site.
